Preheat oven to 350 degrees F and grease 4 individual ramekin dishes. Place the dishes in a large deep sided baking dish and fill the pan with water. Fill about 2 inches up the side of the pan.
Combine all dry ingredients in a medium mixing bowl. Melt the butter and set aside. Whisk the vanilla into the milk. Make a well in the center of the dry ingredients and add the butter and milk. Gradually pull the dry ingredients into the milk mixture in the center and mix until completely combined. Pour the mixture evenly into your four prepared dishes.
For the top layer, combine all the ingredients and stir until it becomes a chocolate liquid. Pour over the chocolate mixture in each individual dish.
Bake for about 25 minutes, or until a toothpick inserted into the top comes out with just a few crumbs on it.
